Indications/Uses
Zollinger-Ellison syndrome. Pantokem Reflux oesophagitis in adults & adolescent ≥12 yr. Eradication of H. pylori in combination w/ appropriate antibiotic therapy in patients w/ H. pylori associated ulcers; gastric & duodenal ulcer & other pathological hypersecretory conditions in adults. Pantokem IV Aspiration syndromes, dyspepsia, GERD, PUD.
Dosage/Direction for Use
Pantokem Reflux oesophagitis in adults & adolescents ≥12 yr 1 tab daily, increase to 2 tab daily. Eradication of H. pylori in adults Pantoprazole 40 mg + amoxicillin 1,000 mg + clarithromycin 500 mg to be taken BID or pantoprazole 40 mg + metronidazole 400-500 mg (or 500 mg tinidazole) + clarithromycin 250-500 mg to be taken BID or pantoprazole 40 mg + amoxicillin 1,000 mg + metronidazole 400-500 mg (or 500 mg tinidazole). Treatment of gastric ulcer 1 tab/day, may be increased to 2 tab. Zollinger-Ellison syndrome & other pathological hypersecretory conditions 2 tab daily. Hepatic impairment 20 mg daily. Renal impairment 40 mg daily. Pantokem IV 1 vial/day over 2-15 min. Reconstituted w/ 10 mL physiological NaCl soln. May be administered directly. Soln must be used w/ in 24 hrs of prep.
Administration
Should be taken on an empty stomach: Swallow whole 1 hr before a meal. Do not chew/crush.
Special Precautions
Hepatic impairment. Not recommended w/ atazanavir. Increased risk of GI infections caused by Salmonella, Campylobacter & C. difficile. Pregnancy & lactation. Pantokem May reduce absorption of vit B12; increased risk of fracture; hypomagnesaemia. Childn >12 yr. Pantokem IV Subacute cutaneous lupus erythematosus. May interfere w/ CgA level. May mask symptoms & delay diagnosis.
Adverse Reactions
Headache, diarrhea. Pantokem IV Pruritus, dizziness, fatigue, constipation, nausea & vomiting, flatulence, abdominal pain, arthralgia & myalgia, urticaria, & dry mouth.
Drug Interactions
May reduce the absorption of ketoconazole, itraconazole, posaconazole & erlotinib. Reduction of bioavailability of atazanavir. Monitor PT/INR w/ phenprocoumon or warfarin. Increased MTX levels in some patients.
